In pregnancy with thyroid disorders, close monitoring of thyroid status uses both TSH and free T4 because pregnancy alters thyroid physiology in ways that TSH alone can miss. TSH reflects pituitary signaling, but placental hormones and changes in thyroid-binding proteins during pregnancy can shift total hormone levels, so measuring free T4 confirms the actual available thyroid hormone. The goal is to maintain a euthyroid state by adjusting thyroid therapy as needed, while also actively managing diabetes. Keeping thyroid function in the normal range supports maternal metabolism and fetal development and helps reduce risks like adverse perinatal outcomes. In this context, monitoring perinatal outcomes is part of comprehensive care. Relying only on TSH, skipping thyroid monitoring, or checking thyroid antibodies monthly does not fit the standard approach, because TSH alone may be insufficient, thyroid status can change even with good diabetes control, and monthly antibody checks are not routinely indicated.
